Tournament Registration

Registration: Form
 All teams must register for the tournament at least 1.5 hours in advance of their first match.  Since most teams will be registering on Saturday morning, please allow enough time before your first match for this activity.  All teams must register at the registration tent, in the Elgin Soccer Complex on the midway by soccer field No. 1 (See the map of the Soccer Complex for exact location). Only the manager has to register the team, coaches and players do not need to go to registration. Registration is available from 4:00 pm to 8:30 pm on Friday evening. Registration will open again on Saturday morning at 6:30 am. Bring all applicable information listed below (items 1 to 6) to the registration tent. Early registration is encouraged.

Required Paperwork

USYSA Teams Required Paperwork

  • State Roster (1 copy)
  • Tournament Roster (1 copy)
  • IYSA Medical Waivers (1 copy per player) check do not need a copy
  • Concussion Form ( I copy signed)
  • Travel permit for out of state players issued by home state association (1 copy)
  • Guest Player forms (1 copy per player)
  • Player Passes

US Club Teams Required Paperwork
  • US Club Official Roster (1 copy)
  • Tournament Roster (1 copy)
  • IYSA Medical Waivers (1 copy per player), need a copy

1) Tournament Roster:
   You must complete the tournament team roster; all the information must be complete. Please print all the information clearly and accurate. A copy of the form is on the web page.
2) State Roster:

 You need to provide a copy of your team’s official state roster at registration, we will keep the copy.

3) NT Form:

For any team that is not part of the IYSA you need to submit this form. A copy is on the webpage in forms and download

4) IYSA Medical Release Form:
You must SHOW us a copy of your medical form.  NEW WE ONLY NEED A COPY IF YOUR PLAYER GETS INJURED AT THE TOURNAMENT. No player will be allowed to participate without a signed form, it must be an IYSA medical form and no substitutes will be accepted! A copy of the form is on the web page.

5) Concussion Form
You need to complete a form for every team. A copy is on the webpage in forms and download

6)  Guest Player Release Form:  
This form must be filed at registration for any guest players. No guest players will be allowed to play without this signed form and a valid player pass. NO EXCEPTIONS! A copy of the form is on the web page.

7) USYSA Player Passes:
   Each player must have a current (2015-2016) USYSA player pass showing a current picture, birth date, year issued and name of team, as specified by the USYSA. Player passes will be checked at the registration tent and at each game by the field marshal. Players can be rostered on ONE team and play for only ONE team. Any player without a pass will not be allowed to play. THERE ARE NO EXCEPTIONS!

8) Travel Permit: 

For any teams outside the State of Illinois, completed and approved Travel Permission forms must be provided. All out of state teams must fax their travel permit prior to the tournament to Jeff Walter at 847-697-9118. Tournament Rules:
 The tournament rules are on the web page, please have your coaches read the rules prior to the tournament. Coaches Note: Coaches and players must be at your assigned field 30 minutes before your match.

Elgin Sports Complex:

Attached to the web page is a map to the Elgin Sports Complex and Verbic Soccer Complex. Ten fields for 7v7 and 9v9 matches will be set-up on the outfields of several adjacent baseball fields.  All the fields are located immediately adjacent to each other and a short walk to the midway.

Alternate Fields:

  In case of delays in the schedule or postponement of some games, several alternate fields have been prepared for tournament play.  These fields are located off site, but only a short drive away.  Abbott Field, for 11 v 11 matches is approximately ½ mile north of the Sports Complex.  College Green Field, for 8 v 8 matches is approximately ½ mile south of the Sports Complex.  Directions to these fields will be distributed if necessary.  Please pray for sunshine!!!

Parking: No additional charge for parking:
 Ample parking (over 1,000 spaces) is available in close proximity to most fields. As a result, no reserved parking for coaches will be provided. See the map for the nearest available parking for your matches.


Several vendors will be at both Midways this year. We will be grilling hamburgers, Brats & hot dogs; we have BBQ, soda, water, sport drinks, chips, candy, and coffee.


 This year we will have a speed shot from Snickers, Chicago Fire, and several other games for the players to participate in, between games.

Team tents:
Limited space for team tents is available. The area for team tents is by Field # 21. Make sure your tent is staked down.

Warm-up areas:

Space is limited so please be considerate to the teams who are playing, please stay away from behind the goals.  For the U-8 to U-11 teams feel free to use the adjacent softball fields. To protect the goal area from unnecessary wear and tear, we have determined that the use of the goal mouths between matches is prohibited. Your cooperation is appreciated.
